This stallion has been registered as a breeding stallion with the USPHA.
The undersigned has received and reviewed the rules governing the use of shipped fresh
or frozen semen in Horses and agrees to abide by them.
The USPHA is hereby released from any responsibility or liability whatsoever in regard to this program.
